The Registry is designed to preserve Washington State residents’ final wishes by recording your advance directives (Living Will and/or Durable Power of Attorney for Health Care), as well as other end-of-life documents, such as the Physician Orders for Life Sustaining Treatment (POLST) form and Mental Health Advance Directives.
