How to Order Patient Materials

This online order form is for WSMA members only, or for those ordering on behalf of a WSMA member. (If you are ordering on behalf of a member, you will need to supply the member's name.)

Patients may order single copies of patient materials by sending us a self-addressed, stamped envelope, for each copy requested, to: WSMA, 2033 Sixth Avenue, Suite 1100, Seattle, WA 98121, Attn: Patient Materials. Please indicate which patient material item you are requesting.
