POLST & Your Practice: Tools to Improve Patient Conversations
Talking to patients and families about code status and the Portable Orders
for Life-Sustaining Treatment (POLST) form can be challenging.
Furthermore, physicians and practitioners rarely receive training on POLST
nor how to approach these discussions with their patients. In this
one-hour webinar, speakers Mark Beiter, DO, Hilary Walker, and Hope
Wechkin, MD, will provide an overview of the POLST form, discuss ways to
identify patients who would most benefit from a POLST conversation, and
introduce a five-step framework for having these discussions.
